Office Hours
Monday through Friday, 8:15 AM to 5 PM. We are closed between 12:30 PM and 1:30 PM for lunch and after 12:30 PM on Wednesdays.

Appointments
When calling for an appointment, please make sure our receptionist has enough information to schedule the proper amount of time. This is particularly important if you have a large number of problems to be addressed during your visit. If you do not have an appointment, please contact us prior to dropping in. We strive to give sme day appointments whenever possible, but we are not an urgent care facility.

Routine lab visits, blood pressure checks, injections, and other nurse related appointments should be scheduled with the receptionist to help reduce your waiting time.

Please bring your medical insurance card(s), co-pay and all necessary paperwork to your appointment. Please update us if there is a change in your medical insurance to avoid problems in billing.

We strive to be on time and are happy to reserve your appointment time just for you. Please be punctual for all appointments. If you cannot keep your appointment, please call 24 hours ahead to allow us to offer that time to another patient. New patients should arrive 15 minutes prior to their appointment time, in order to complete registration. For your convenience you may download registration materials from this site, or pre-register at the office.

Prescriptions and medication refills
An accurate medication list is an important part of your health record. Please bring your medicines with you for each appointment.

Please allow 24 hours notice for Rx renewals during normal office hours. Limited Rx renewals on weekends and holidays are done at the discretion of the covering physician. Please do not wait until you are out of a medication before calling for refill.

Medical records
If you are new to the office and/or have outside medical records pertinent to your visit please bring them with you or notify our office ahead of the appointment so that we may request records prior to your visit.

Patient privacy is a priority in our office. If the patient requests disclosure of Protected Health INformation to outside persons or entities, the patient must first give written authorization for such disclosure.

Telephone calls
Occasionally our phone lines will be busy with other patient calls. If this occurs you will be connected with our answering service. Please give their operator our name, telephone number, and a brief message. We will return your call promptly.

Please turn off your cell phone while being seen by the doctor.

If you need to be contacted by the physician after hours please contact the on call physician by dialing (360)-297-4911 which will connect you to the answering service. Please disable caller ID, and make sure you are available when your call is returned.

Insurance, fees and payments
Charges for office visits are determined by the complexity of your poblem and the level of service porvided. Our fees are adjusted annually, and if applicable, by the specific caontract we have with an insurer. Please call our office manager for questions regarding insurance or billing.

We recommend each patient contact their insurance company to verify coverage. Many insurers now require us to collect co-pays. All co-pays are due at the time of service. We will file insurance claims for either private or government insurers, but the patient is primarily responsible for making sure the medical services are paid for. Any portion of a charge not covered by insurance is the patient's financial responsibility.
